What is Hospice Care?

Hospice Care is appropriate when curative treatments are no longer available, working, or desired by an individual, and supportive services are being sought as the individual continues their life’s journey to the end.

The Faith & Family Supportive Care & Hospice team provides hospice care wherever our patients call home—whether a private residence, a senior living community, assisted living, or a long-term care facility.

The Hospice Team includes:

Specially trained Hospice Physicians and Nurse Practitioners: Our skilled doctors and practitioners oversee all the care we provide, and there is a physician on call 24/7 to ensure we meet our patient’s changing needs as quickly as possible, day or night

Registered Nurses and Licensed Practical Nurses: Our nurses visit as often as needed to assess our patients and manage their symptoms. We educate caregivers on the dying process so that families and caregivers understand what is happening and what to expect, as well as how they can best support their loved ones as things change. We have a nurse on call at all times to address questions and needs as they arise.

Certified Nursing Assistants/Aides: Our skilled and compassionate hospice aides help with bathing and other activities of daily living, while providing companionship and support.

• Social Workers
: Our medical social workers are a valuable resource to our patients and care team in identifying community resources that may benefit our clients, as well as offering direct emotional support to patients and families throughout their hospice journey.

• Chaplain
: Our chaplain is available to every patient and family to directly provide or connect them with needed spiritual support during what is often a challenging and complicated season of life.

• Volunteers:
Our volunteers assist with vigil sittings at end of life, to be there when families cannot be.
